Required Documentation

 

*Note: All DEERS eligible customers must show two valid forms of ID (see Valid Forms of Identification) required documents mandatory for an ID card and/or Common Access Card (CAC) all documents must be original or notarized copies only.

General Information/Services Provided:

  • Identification (ID) Cards

    • Two forms of ID required for sponsor, spouse, and dependent children over the age of 21

    • Dependents that are 21 and older must have college letter stating they are full-time students. The letter must include a projected graduation date from an accredited institution.

    • If spouse is deployed, the civilian spouse must have a Power of Attorney to get an ID Card.

    • Lost ID Card: Ranks E4 and below must bring a signed Lost ID letter (by the First Sergeant or Commander) to the Customer Service Desk, Bldg 5616, Rm 131.

  • Name Changes: Original/certified copies of marriage certificate/divorce decree (in English) are required.

  • Enroll New Born Baby: Original or certified copies of birth certificate.

*Note: Legal Assistance is a service provided by the 802 MSG/JA office on an appointment basis only for Active Duty, Dependents and Retirees; Reservist/Dependents (Must be on order).

 

 

In-Processing

Location: Building 5616, Room 108

Hours of Operation: 0730-1630, M-F

 

General Information:

  • Inbound personnel must report to Bldg 5616, Room 108, with a copy of PCS orders before going on permissive TDY.

  • Once assigned (gained) to the base, members will be scheduled for “RIGHT START.”

    • Bring orders, a pen, and all documents provided by previous base when you final out-processed.
  • DV in-processing (O-6 and above/Command CMSgts)—FMO will schedule one-on-one appointments.  (Note: Due to the substantial number of Colonels assigned to LAFB, only senior officers billets previously identified are entitled to DV in-processing.)

  • For Accessions, if this is your first duty station, stop by the Force Management office with a copy of your EAD orders and all other accession paperwork.

 

Leave Program

Location: Building 5616, Room 106

Hours of Operation: 0730-1630, M-F

 

General Information/Services Provided:

  • Submit ordinary leave requests in LeaveWeb NLT 3 duty days prior to departure.

  • Advanced, Convalescent, and PTDY are processed on AF Form 988 with commander's signature. Scan and send this form to 802 FSS Force Management Ops org box or drop it off for a leave number prior to departing on leave.

  • For emergency leave contact your unit's First Sergeant.
